Application for launching a new course of study includes, inter alia:
description of the conditions for conducting studies, including a list of academic teachers and other persons proposed to teach – general academic profile attachment No. 8 to the JKU Rector's Order No. 22/2020 as amended or practical profile attachment No. 9 to the JKU Rector's Order No. 22/2020 as amended. -
The application for permission to create a study course also includes, inter alia:study program attachment No. 2 to the JKU Rector's Order no. 22/2020 as amended.
- schedule of implementing the study program attachment No. 3 to the JKU Rector's Order no. 22/2020 as amended;
- list of courses ... with a general-academic profile attachment No. 6 to the JKU Rector's Order No. 22/2020 as amended or with a practical profile attachment No. 7 to the JKU Rector's Order No. 22/2020 as amended;
- syllabuses – in Polish attachment No. 4 to the JKU Rector's Order No. 22/2020 as amended or in English attachment No. 5 to JKU Rector's Order No. 22/2020 as amended;
- copies of declarations of academic teachers concerning their employment at JKU attachment No. 10 to JKU Rector's Order No. 22/2020 as amended and copies of declarations of other persons concerning their employment at JKU attachment No. 11;
- copies of agreements with employers or employers' declarations concerning the acceptance of students for placements (p. template);
- opinions of external stakeholders concerning the study program (p.wzór).
The application, together with a copy of the Faculty Council's resolution on expressing an opinion concerning the application for creating a new study course, should be submitted to the Vice-Rector for Education in paper form and in electronic form to the Education Quality Section.
Note! In the case of signed documents, their scans must be attached.
