Health insurance for students over 26

Pursuant to the Act on health care services financed from public funds of 27 August 2004 (i.e. Journal of Laws of 2022, item 2561) a student up to the age of 26 must be enrolled for health insurance by his or her parents, or, if no parent is subject to health insurance, by the grandparents.

1. The University may enrol a student in health insurance if he/she fulfils the following conditions together:

- is 26 years old,

- does not work under a contract of employment, a contract of mandate, etc.,

- has no other title to health insurance (e.g. with a spouse, from an internship).

In order to register for health insurance, the student should go to the Student and Doctoral Affairs Department (ul. Żeromskiego 5, 25-369 Kielce, room 16) in order to fill in the documents: application for health insurance coverage and ZUS ZZA form, or deliver the above-mentioned documents by post.

IMPORTANT

  • Students who have registered for health insurance at the University and completed the first cycle (submitting the diploma thesis and passing the diploma exam) lose the right to health insurance and are subject to deregistration on the day of graduation. In accordance with art. 67 point 5 of the Act on health care services financed from public funds of 27 August 2004, the student's right to health care services expires after 4 months from graduation.
  • According to the above, graduates of the first cycle programme who are entering the second cycle programme and wish to be covered by health insurance by the University should submit a new application for health insurance coverage after matriculation, i.e. after 1 October.

The insured student can also insure members of his/her family (non-working wife, non-working husband and/or children).

Required documents (to be inspected):

  • identity card of the spouse,
  • marriage certificate,
  • the child's birth certificate,
  • the child's PESEL number.

2. Foreigners holding a Pole's Card or a certificate from a Consul confirming Polish origin - health insurance

Students with a valid Pole's Card or a certificate from the Consul stating that they are of Polish origin will be able to receive medical care on the same basis as Polish students. In order to benefit from health insurance, formalities must be completed.

  • For this purpose, the student reports to the Student and Doctoral Affairs Department with the following documents:

- A Pole's Card or a certificate from the Consul confirming Polish origin;
- Completed application for health insurance coverage (printable in the annex above);
-Completed ZUS ZZA form, which should be filled out according to the instructions (downloadable forms in the attachment above).
 

Students who are citizens of the European Union in the absence of a title to health insurance in the country may be notified by the university. For this, please contact us by phone or via email.

3. Non-EU students

Students who are not citizens of European Union countries and who do not have a valid Pole's Card or a certificate of Polish origin from a Consul are subject to voluntary health insurance.

  • For this purpose, the student reports to the National Health Fund to sign a voluntary agreement on the basis of the following documents:

- certificates of study;
- certificate of residence/registration in the Świętokrzyskie voivodship;
- document confirming the right to stay in the territory of the Republic of Poland.

  • The student then delivers the signed contract to the Social Insurance Institution, on the basis of which he/she is registered for health insurance.

He/she pays health insurance premiums on his/her own by making a monthly payment to the individual ZUS subaccount in the amount of 55.80 PLN.
 

A student reporting to a health care facility must present:
- the application for health insurance (ZUS P ZZA form);
- the contract with the National Health Fund;
- the confirmation of contributions paid.
